So, yeah, the general feel of it is: We want someone who issues a challenge to basically incite the other Strain 117s into a grand melee duel, keeping in mind that they can chose to spend sanity (and if they're at 1 sanity, go into their derangement) to avoid it. So, really, you don't NEED to respond to the challenge as a player... as long as you accept the consequences.
And, you spend 1 sanity for each call. If someone else decides to respond to the challenge with a challenge, that's 2. But, no, no matter how many are present, the spend is in response to avoiding the challenge.
The other thing is: The challenge only makes you fight. Mike is correct... We don't force you to suck someone's soul out (which is what you do with the annihilation call). That is all on your PCs conscience. And, remember: Dark games are all about consequence. Those who seek power through murder and death often are marked and will eventually reap what they sow.
